NOTES: The USS Chicago was originally commissioned as CA-136, an all-gun Baltimore class cruiser in 1945. She was decommissioned on 6 JUN 1947. The ex-Chicago was selected for extensive modification in 1958 and converted to an all-missile Albany class cruiser. Her hull designation was changed to CG-11 on 1 NOV 1958 and was re-commissioned on 2 MAY 1964.
